Carteret Health Care Home Health and Hospice Earn Excellence Awards

Home Health & Hospice earned the 2023 SHPBestTM “Superior Performer” Patient Satisfaction Award for Home Health and Caregiver Satisfaction Award for Hospice

September 2024 – Carteret Health Care Home Health & Hospice has been recognized by Strategic Healthcare Programs (SHP) as a “Superior Performer” for achieving an overall patient, caregiver and family satisfaction score that ranked in the top 20% of all eligible SHP clients for the 2023 calendar year, in both the Home Health and the Hospice service lines.

The annual SHPBest™ Award Program was created to acknowledge Home Health and Hospice providers that consistently provide high quality service to patients, families, and caregivers of patients receiving home health and hospice care.

For Home Health, the 2023 award recipients were determined by reviewing and ranking the overall satisfaction score for more than 3,200 home health providers. For Hospice, the 2023 award recipients were determined by reviewing and ranking the overall CAHPS Hospice survey satisfaction score for more than 1,750 hospice providers. With one of the largest CAHPS benchmarks in the nation, SHP is in a unique position to identify and recognize organizations that have made patient, family, and caregiver satisfaction a priority and have been rewarded for their efforts with high marks on the CAHPS surveys.

Carteret Health Care Home Health & Hospice offer a team approach to care, with the patients and the caregivers at the center of the team. As part of Home Health, patients can receive skilled nursing, physical therapy, occupational therapy, speech therapy, aide services, dietitian services and social work support. The goal is to help our patients and families achieve their highest level of health so that they can remain in the community and enjoy a happy and healthy future.

With Hospice, patients and families can receive all of the above services with the addition of Chaplain services, Volunteer Services, and Bereavement outreach and support for 13 months following the patient’s passing. Hospice’s goal is to honor the patient and their wishes, and to provide care and support to enable them to stay in their home surrounded by loved ones with the best quality of life and comfort as they travel the end of life journey.
