Meet Our Community Health and Outreach Coordinator

Teresa Schray, BSN, RN, VA-BC is a seasoned RN with over 30 years of diverse nursing experience and is now Carteret Health Care's Community Health and Outreach Coordinator. She champions community outreach and education, focusing on chronic diseases such as congestive heart failure and diabetes. Teresa has led impactful projects on hospital readmissions and developed interactive educational programs for patients and families. Teresa was recognized for her work and was a guest speaker at the American Heart Association Scientific Sessions in 2023. Passionate about enhancing patient outcomes and community well-being, Teresa is committed to empowering individuals with the knowledge and tools to lead healthier lives.
